You specified organised competitive matches. Vintage is almost never played in a tournament format outside a few specific conventions. Organised tournaments for EDH are also pretty rare, unless there happens to be a venue in your area that runs a league.

primarily that the wounds get shuffled in and aren't publicly tracked. I've seen people lose plenty of games of Nightfall they could have won if they'd been keeping count and attacked the right people in the last few turns :)

Also note the impetus is always on the controller of the ability. For example with a Frost Titan. If the person who casts a spell forgets to pay an extra (1), the spell is not automatically countered. The Titan's controller has to remind them if they want the trigger to apply.

The answer to the question in the title is actually no. A player loses the next time state based actions are checked after they reach 0 or less life. This rarely matters but it means you could, for example, cause a draw by targeting your opponent and then yourself with Arc Trail, even though it deals damage to them before you :) //useless-nitpick

He's not completely doomed, there are cards, e.g., Stifle, One With Nothing, that could still save him, but Disenchant isn't gonna do the job.

This is a judging question. As with all judging questions there is no single absolutely correct answer. It will depend on if the judge feels seeing the extra card has any relevance to the decision to untap. (And if they actually looked at the card or just had it in their hand because they reached across the table to do everything at once.) (And to some extent REL even though it really shouldn't matter.)

Cannot tell from the picture how serious the damage to the lotus is. That could be a mild crease or it could be a full tear that's been repaired, which could take several hundred dollars off the retail value. The other two, if you look closely, have square corners, they are collector's edition cards, not "real" magic cards, worth maybe $50 each at most if you're a retailer that can find buyers interested in them. Given the incredible hassle of parting out an retailing a collection, I doubt he got ripped off, unless there were playsets of dual lands in those "uncommons."

There is only one stack. It is a game zone. Spells do not "start a stack." It's always there. Mixing terminology with Yu-Gi-Oh chains is a bad path to follow :)

This is incorrect. The first check of the if condition occurs when the trigger condition is met, not when the ability would be put on the stack. These are not the same time. It matters for spells like Hypergenesis where things are not entering the battlefield simultaneously.
